Bipolar disorder · Bangladesh
Intense swings between emotional highs and deep lows can be confusing and exhausting — for you and those around you. Bipolar disorder is manageable with the right combination of care. Work with licensed professionals online nationwide, or in person in Dhaka.
In plain language: Bipolar disorder involves shifts between highs (mania or hypomania) and lows (depression) that go beyond normal mood changes. It is best managed with medication from a psychiatrist plus therapy and lifestyle support — together, most people live full, stable lives.
The basics
Bipolar disorder is a mood condition marked by episodes of unusually high mood (mania or hypomania) and episodes of depression.
During a high, a person may feel euphoric or irritable, need little sleep, talk fast, take risks, or feel invincible. During a low, they experience the heaviness of depression — exhaustion, hopelessness and loss of interest. Between episodes, mood can be stable. These are not ordinary mood swings; they are more intense, last longer, and disrupt life. Bipolar disorder has a strong biological basis and needs medical treatment, but therapy and support are essential too. In Bangladesh, it is often misunderstood or diagnosed late. With the right care it is very manageable, and this is a health condition, not a character flaw.
How bipolar disorder shows up
Bipolar disorder is not quick mood swings within a day. It moves through episodes of high and low mood that usually last days to months, with steadier periods in between.
Based on the clinical description of bipolar I and II disorder (DSM-5). Illustration, not a real mood record.
How it shows up
Elevated or irritable mood, racing thoughts, little need for sleep.
Impulsive spending, decisions or behaviour during highs.
Deep sadness, exhaustion, hopelessness and loss of interest.
Marked changes in energy, activity and functioning.
Sleeping very little in highs, too much in lows.
Episodes that disrupt work, study and relationships.
Mood that shifts with life events is normal. Bipolar disorder is different: highs and lows that last days or weeks, with changes in sleep, energy and behaviour that people around you notice. It is often first diagnosed as depression, so if you have had depression and also periods of unusual energy, very little sleep or risky decisions, tell your psychiatrist or psychologist. Never stop or change bipolar medication without your psychiatrist, and protect a regular sleep routine, as losing sleep can trigger a high.

Bipolar disorder is best managed with a combination of medical and psychological care.
Medication prescribed by a psychiatrist (such as mood stabilisers) is the foundation of treatment and helps prevent episodes. Alongside this, CBT and psychoeducation help you recognise early warning signs, manage stress and triggers, keep steady routines and sleep, and stay well between episodes. Family support helps loved ones understand and help. This combined approach lets most people live stable, full lives — therapy is not a replacement for medication, but a powerful partner to it.
What to expect
Proper assessment and medical treatment from a psychiatrist.
Spotting early warning signs of highs and lows.
Routines, sleep, stress management and coping skills.
Ongoing support to prevent relapse and live fully.
If you or someone you know is in a severe manic or depressive episode, at risk of harm, or thinking about self-harm, treat it as a medical emergency. Call 999 or go to the nearest hospital.
