Psychologist vs Psychiatrist: What’s the Difference?

Short answer: A psychologist treats mental health problems with talking therapy, such as CBT, and does not prescribe medicine. A psychiatrist is a medical doctor (MBBS plus specialist training in psychiatry) who diagnoses mental illness and can prescribe medicine. For stress, anxiety, low mood, trauma or relationship problems, most people start with a psychologist. See a psychiatrist first if symptoms are severe, you cannot manage daily life, or there are hallucinations, mania or thoughts of self-harm.

In an emergency, call 999 or go to the nearest hospital emergency department.

If you are looking for mental health support in Bangladesh, one question comes up again and again: should you see a psychologist or a psychiatrist? The names sound alike and the work overlaps, so it is easy to book the wrong one. This guide explains the difference in plain language and helps you decide where to start.

What does a psychologist do?

A clinical or counselling psychologist has a degree in psychology and postgraduate training in assessment and therapy. In Bangladesh, clinical psychologists usually hold an MS and MPhil in Clinical Psychology, for example from the University of Dhaka. They do not prescribe medicine. Instead they work with you over a series of sessions, using structured talking therapies such as Cognitive Behavioural Therapy (CBT), to understand what is happening and give you practical tools to manage it.

Psychologists help with anxiety, depression, trauma, OCD, grief, stress, and relationship and family problems. They can also carry out psychological assessments, such as for ADHD.

Bangladesh does not yet have a government licence or registration system for psychologists; researchers writing in The Lancet Psychiatry in 2024 called for one (Lancet Psychiatry). Until then, check a psychologist’s degrees and their membership of a professional body such as the Bangladesh Clinical Psychology Society (BCPS).

What does a psychiatrist do?

A psychiatrist is a medical doctor (MBBS) who has specialised in mental health, usually with an FCPS or MD in psychiatry. Because they are doctors, they can diagnose conditions from a medical point of view, order tests and prescribe medicine, for example for severe depression, bipolar disorder, schizophrenia or severe anxiety. Psychiatrists must be registered with the Bangladesh Medical and Dental Council, so you can check their registration.

Psychiatrist appointments are often shorter than therapy sessions and focus on diagnosis, medicine and follow-up. Some psychiatrists also offer talking therapy.

Which one should you see first?

Start with a psychologist if you are dealing with stress, worry, sadness, relationship problems, grief, low confidence, or you simply want to talk to someone and learn ways to cope. This covers most people who look for help.

See a psychiatrist first if:

  • you cannot manage daily life, such as work, study, eating or sleeping;
  • you have thoughts of harming yourself;
  • you hear or see things others do not, or hold beliefs others find very unusual;
  • you have periods of very high energy, little sleep and risky behaviour (possible mania);
  • you already take psychiatric medicine and need it reviewed.

If you start with a psychologist and they think medicine could help, a good psychologist will tell you honestly and refer you for a psychiatric review.

Psychologist or psychiatrist for anxiety and depression?

For most anxiety and depression, talking therapy comes first. The World Health Organization says psychological treatments are the first treatments for depression, that they can be combined with antidepressants in moderate and severe depression, and that antidepressants are not needed for mild depression (WHO). For generalised anxiety, the NHS says people are usually advised to try talking therapies, usually CBT, before medicine is prescribed (NHS).

So for mild to moderate anxiety or depression, a psychologist is usually the right first step. If symptoms are severe, have lasted a long time, or therapy alone is not helping, add a psychiatrist.

Do you need both?

Often, yes, when a condition is moderate to severe. Medicine from a psychiatrist can steady symptoms while a psychologist works on the thoughts, habits and situations behind them. The US National Institute of Mental Health notes that psychotherapy can be used as an alternative to, or alongside, medication (NIMH). The two professionals work best when they share a plan, with your permission.

Cost and access in Bangladesh

Psychiatrists are fewer and mostly based in Dhaka, so appointments can take longer to get. Psychologists, especially online, are easier to reach for ongoing therapy.

At Chum Wellness, psychologists charge from ৳1,000 to ৳5,500 a session depending on experience, and university students pay from ৳699 online with selected psychologists. Our four psychiatrists see patients in Moghbazar and Mohammadpur and online, at ৳2,000 to ৳3,000 a consultation; compare them in our best psychiatrist in Dhaka guide or read about online psychiatrist consultations. Every fee is shown before you book, and confidentiality is the same with both: nothing you share is passed to family or employers without your consent.

Frequently asked questions

Is a psychologist or a psychiatrist better?

Neither is better; they do different jobs. A psychologist provides talking therapy, and a psychiatrist provides medical diagnosis and medicine. Most people start with a psychologist and add a psychiatrist if medicine is needed.

Can a psychologist prescribe medicine in Bangladesh?

No. Only a doctor can prescribe medicine. A psychologist treats with talking therapy such as CBT and can refer you to a psychiatrist if medicine may help.

Should I see a psychologist or a psychiatrist for anxiety?

For most anxiety, start with a psychologist: talking therapy, usually CBT, is the first treatment. See a psychiatrist if the anxiety is severe, stops you functioning, or does not improve with therapy.

Do I need both a psychologist and a psychiatrist?

Sometimes. For moderate to severe conditions, medicine from a psychiatrist and therapy with a psychologist together often work best.

How do I check that a psychiatrist or psychologist is qualified?

A psychiatrist must be registered with the Bangladesh Medical and Dental Council (BMDC). For a psychologist, ask about their degrees (such as an MS or MPhil in Clinical Psychology) and professional membership, such as the Bangladesh Clinical Psychology Society.
